This would be caused by inserting symbols as a character string instead of using the html markup. For example to insert an apostrophe (') use 39;

You shouldn't have to use html entities for input.

Likely inconsistencies in the database table encoding and the page encoding, check your tables all have the correct encoding, converting existing content should you need to, and then set your site to output in that same encoding.

Did you remove the <meta charset="utf-8"> from your layout.tpl?
